Paul Simon of Simon and Garfunkel fame wrote “Slip Slidin’ Away” in 1975, a melody about how quickly life can unravel. Be it a lost love, success unrealized, promises not kept, each pales in comparison with the slip-slidin’ of dementia.
Dementia takes away everything we hold dear—memories, relationships, our own life story. Every three seconds, someone receives this heartbreaking diagnosis, making it the most feared condition among those aged 55 years and older. According to the Alzheimer’s Association, Alzheimer’s care in the U.S. costs $345 billion annually. Additionally, 11 million family members and caregivers provided unpaid care valued at $340 billion in 2022, further highlighting the immense financial and personal burden of the disease.
Yet, more than a “slidin’ away,” dementia is a catastrophic earthquake of heart-rending emotions. By 2060, approximately one million Americans could be diagnosed with some type of dementia annually, according to a study published in Nature Medicine. This compares with 514,000 cases in 2022. The study estimated the likelihood of developing dementia after age 55 is 42% and increases significantly to 56% for individuals over 85 years old.
Menopause as a link
Recently, researchers and healthcare officials have been intrigued by the idea of one culprit that complicates the dementia “formula” and may be a link to the disease.
The connection between menopause and Alzheimer’s disease—a form of dementia, like Parkinson’s, Huntington’s, and Lewy Body, for example—was considered in a 2023 study by Mayo Clinic.
Among the results, it was revealed that hormonal changes during menopause significantly impact brain health, potentially increasing the risk of Alzheimer’s in women. Consequences: By 2050, two-thirds of the 13.8 million people in the U.S. likely to suffer from Alzheimer’s will be women.
Understanding this link is crucial for prevention and treatment strategies.

Hormonal changes and brain health
The role of estrogen in cognitive function is foundational, a “master regulator of bioenergetic systems in the brain and body,” says the National Library of Medicine. Estrogen is not only vital for reproductive health but also plays a neuroprotective role.
Estrogen enhances memory function and protects the brain from age-related decline. During menopause, estrogen levels drop, leading to metabolic and structural changes in the brain that may predispose women to neurodegenerative diseases like Alzheimer’s. Other research suggests that an early decline in estrogen increases women’s risk of such health villains as cardiovascular disease, Parkinson’s, depression, osteoporosis, high blood pressure, weight gain, midlife diabetes, and cognitive disorders.

During menopause, estrogen levels drop, leading to metabolic and structural changes in the brain that may predispose women to neurodegenerative diseases like Alzheimer’s.
Early menopause and increased Alzheimer’s risk
Neuroimaging studies reveal that menopause is associated with significant structural and functional changes in the brain. Additionally, early menopause (before age 45) or surgical menopause significantly raises the risk of Alzheimer’s disease. Early menopause retards estrogen’s neuroprotective benefits prematurely, leaving the brain more vulnerable to age-related damage. These changes may explain the increased vulnerability to Alzheimer’s disease.
Estrogen workloads aside, neuroimaging can show a loss of gray matter volume in the brain, and a shrinkage in memory-related brain regions in menopause. A decline in white matter integrity can occur, often disrupting communication across the different regions of the brain. In postmenopausal women, decreased glucose metabolism has been evidenced, and networks aligned with memory and cognitive processing show disturbed connectivity during menopause. (Note: Gray matter and white matter do have different responsibilities, but both are essential for healthy cognition thinking, memory, emotional regulation, and more.)
These findings suggest that menopause may serve as an early biomarker for Alzheimer’s risk, reinforcing the need for proactive brain health strategies.
On a brighter side, all is not lost. Lisa Mosconi, author of The Menopause Brain, has examined the neurological symptoms of menopause, including the new mental skills it can foster, and the options for brain-inclusive menopause care. She writes that menopause can actually enhance emotional stability and self-confidence. Neurologically, a small part of the brain—the amygdale —becomes less reactive to negativity, leading to greater peace and life contentment. Many postmenopausal women also experience increased empathy and feel more comfortable in their own skin.
Prevention and intervention strategies
Given the growing evidence linking menopause and Alzheimer’s, targeted interventions can help mitigate risks.
Healthcare providers agree that regular exercise can reduce the risk of succumbing to Alzheimer’s. Activities like reading, puzzles, and social interaction stimulate brain plasticity—the brain’s ability to modify its connections or rewire itself. Proper diet—think Mediterranean—is associated with better cognitive aging, and, because heart health is related to brain health, management of cholesterol, blood pressure and diabetes risk are important prevention and intervention strategies.
Given the growing evidence linking menopause and Alzheimer’s, targeted interventions can help mitigate risks.
